Most vision plans cover the basics — a routine eye exam, a standard frame allowance, and either eyeglass lenses or contact lenses. However, there are some common exclusions to be aware of:
Lens upgrades such as anti-reflective coatings, blue light blocking, or progressive lenses may only be partially covered or require an out-of-pocket upgrade fee
Premium frames above your plan’s allowance will require you to pay the difference
Medical eye care visits — if your visit is for a medical condition such as dry eye, glaucoma, or diabetic eye disease, it may be billed to your medical insurance rather than your vision plan
Cosmetic procedures are not covered by vision insurance
